Experienced Urologist Focused Exclusively on Trans Masculine Bottom Surgery

Dr. Michael Safir, MD is a board-certified urologist in San Francisco who specializes exclusively in trans masculine bottom surgery procedures such as Phalloplasty and Metoidioplasty. With subspecialty certification in Female Pelvic Medicine and Reconstructive Surgery (FPM-RS), Dr. Safir is one of a handful of urologists in the world with individual fellowship training and experience in both male and female genital reconstruction. Dr. Safir’s vast experience in urology and compassion for his patients has made him a popular choice among trans men seeking bottom surgery on the West Coast.

Dr. Safir earned his undergraduate and medical degrees at Northwestern University. After completing his urology residency, also at Northwestern, Dr. Safir completed two year-long fellowships in Male (UCSF) and Female (UCLA) Genitourinary Reconstructive Surgery. Dr. Safir has served as Section Chief of Reconstructive Urology at New York Medical College and as a Clinical Instructor in Urology at both UCLA and UCSF. Before joining the Crane Center for Transgender Surgery in San Francisco, Dr. Safir practiced urology in Los Angeles, where he served as President of the Los Angeles Urological Society.

Dr. Safir is board-certified by the American Board of Urology. He is a member of the World Professional Association for Transgender Health (WPATH.)

Dr. Safir is a frequent presenter at transgender conferences in the United States. He will be presenting at Trans Wellness (July 25-28) and Gender Odyssey (Aug 1-4) and offering free on-site consultations. More information about registering for a consult can be found on the Crane Center website.

Dr. Safir performs the following Gender Affirming Surgery procedures:

  • Phalloplasty: Forearm (RFF) and thigh (ALT) flap. Options include Urethral Lengthening, Vaginectomy, Scrotoplasty, Glansplasty, Testicular and Penile Implants.
  • Metoidioplasty: Options include Urethral Lengthening, Vaginectomy, Scrotoplasty, Testicular Implants, Monsplasty.
  • Reconstructive Urology: Fistula and stricture repair.

Dr. Safir operates at California Pacific Medical Center – Davies Campus and St. Francis Memorial Hospital in San Francisco, and Greenbrae Surgery Center and Bon Air Surgery Center in Greenbrae.

Surgery Requirements

Dr. Safir follows WPATH guidelines for surgery:

  • You will need to provide two referrals from qualified mental health professionals; one referral for other procedures unless otherwise specified by insurance.
  • A letter from your hormone specialist stating you have been on 12 months of continuous hormones, unless clinically contraindicated.
  • Dr. Safir will operate on minors with parental consent.

Financing & Insurance
Dr. Safir accepts credit/debit cards, cashier’s check, money order, wire transfer and medical financing via CareCredit. He also accepts insurance and is in-network with Anthem Blue Cross, Blue Cross Blue Shield, Blue Shield of California, Sutter Select, Western Health Advantage, and San Francisco Health Plan. The Crane Center has also had success billing out-of-network with Aetna, Group Health, Harvard Pilgrim, Health Net, HealthPartners, Medica, most managed Medi-Cal plans, Pacific Source, Sharp Health Plan and United Healthcare. Medicare is not accepted.

Booking a Consultation
Dr. Safir is available for phone or in-person consultations in San Francisco for a fee of $300. An in-person consultation is not required for booking a surgery date.
